She knew how to take care of physical damage, but she is clueless about soul and here in 100 kingdoms, everything and everyone is too low level to help her.

Qin Chen also isn't never ill. In fact his previous life he was permanently handicapped and stuck in level. This is why he knows medicine so well. Basically, whole previous life was used to seek cure for his ailment. Even in this life he starts with same problem, but the difference is that when he figured it out in previous life it was already too late for the method

My only (forced) explanation I could possibly give for him is... maybe... possibly... all the good and proper cultivation methods got lost in time and only partial or foulty ones remain and now when you get strong enough there is always going to be something wrong but since they don't know any better they don't realise it's a problem because "it's always like that".

While he was looking for cure for his problems in previous life, he pretty much scoured all forbidden areas, not to mention that he was creating new skills in hope to find one that would help him. He was actually known as the one who explored the most. And his personal skills? His two enemies are pretty much all built on his skills he created for them before betrayal

And some of the methods and items were actually laid in his path for him to find. But, I won't spoil it as this concerns whole lot of main side story
